What this is, and who it is for
The Form Library keeps your organization's reusable form designs so that a standard Demographics or Vital Signs is drawn once and reused in every study. This page is the task: put a form in the library, and use one in a study. What a template is, what is copied and what the library does not have are on The form library and templates.
| You need | To |
|---|---|
form.read or form.design | See Form Library in the menu and read templates |
library.write | Create, change and remove templates |
form.design or study.write in the study | Use a template in a study |
Held by: Organization Admin, eCRF Designer (library.write).
Where to find it
Form Library in the main menu. The Form Templates tab lists templates (grouped by therapeutic area with Group by therapeutic area, or as a table of code, name, category, phase, tags and field count) with a search. An empty library reads No form templates yet.
What you can do
Put a form in the library
- New form template: enter the code, name, description, category, CDISC domain tag, therapeutic area and phase (Create & open builder takes you to the builder to draw it), or
- Add from study: choose a study, tick the forms to promote and confirm; a form whose code is already in the library is skipped.
- Copies the ticked forms into the library as templates.
Use a template
- In the library, a template's Use in study: pick the study (search by code or title) and confirm. The template is copied into the study's version in Build as a new Draft form, you are taken to that study's Forms list, and the link back to the library is kept. If the template is an event form you are asked what kind of event it is.
- In a study, Forms → Import from Library: the same copy from inside the study.
- In the builder, Import: append only the template's fields to the form you are editing (The form builder).
Change or remove a template
A template's menu has Open in builder, Edit details and Remove. Changing a template does not change forms already copied into studies, and Remove archives it: it leaves the list and is kept.
Every option
See The form library and templates for the fields of a template, what is copied, and the four library tabs that are not built (Variables Library, Visit Templates, Codelists, Saved bundles).
What the system records
Creating, changing and archiving a template is library_form.changed. Copying into a study is the study's form.created with source library.
What you cannot do, and why
- Keep a study's form in step with its template. The copy is independent.
- Edit a study's copy in the library. Promote it again as a new template.
- Use a template in a study with no version in Build.
- Import the same template twice into one study (is already in this study).
Compliance Q&A
